Vision Plan

We offer a vision plan that covers benefits for eye exams, lenses and frames and contacts after a copay. If you see a doctor within the network, you will not have to file any claims. If you see a doctor outside the network, you will receive reduced benefits, and have to pay at the time of service and then file for reimbursement. Each year this plan pays the full cost (after copay) for one vision exam, one pair of lenses and frames, or a specified amount towards the cost of contact lenses (instead of lenses and frames). Some lens and frame options will require you to pay a portion of the cost of your glasses.
